PRIVACY PROTECTION POLICY

It is the policy of Westview Health Care Center that all information pertaining to patients and residents medical and financial information is held in absolute strict confidence.

At no time shall any employee or volunteer be authorized to share protected information as defined herein.

Any person who collects Social Security numbers in the course of business shall adhere to this policy.

The policy is intended to:

This policy further requires any person in possession of any "personal information" about another person to safeguard such information, including any computer files or documents containing it, from misuse by third parties, and to destroy, erase, or make unreadable any document, computer file, or data before disposing of the information.

"Personal information" means information capable of being associated with a particular individual through one or more identifiers, such as a Social Security number, driver's license number, state identification card number, account number, credit or debit card number, passport number, alien registration number, or health insurance identification number, and does not include publicly available information that is lawfully made available to the public from federal, state, or local government records or widely distributed media.
